At The Canadian Hearing Society (CHS) we respect your personal health information and are committed to client confidentiality, in accordance with the Ontario Personal Health Information Protection Act and all other applicable laws.

We are committed to protecting the privacy and security of the personal information of our consumers, volunteers, staff and donors.

We collect your personal information directly from you or from the person acting on your behalf.  That information may include: your contact information, medical history, and record of past services.   

Uses and Disclosures of Personal Health Information


CHS may use and disclose your personal health information to:

• Provide you with the best possible service, now and in the future
• Receive payment from OHIP, WSIB, ADP, your private insurer or others
for the services we provide to you

In addition, we may share your contact information – just your name, address, email, phone/TTY – with our communications and fundraising departments, so that you can receive our magazine Vibes, as well as information about upcoming CHS events and other relevant information.  Fundraising helps to ensure the ongoing stability and quality of the services and programs CHS provides to deaf, deafened and hard of hearing individuals and their families.
